1. To apply for membership in JSDT, send a request for an application form to JSDT by fax or snail mail (postcard only).

2. To discontinue membership temporarily or permanently or to change your profile, complete a card attached to the odd-numbered issues of the Journal of Japanese Society for Dialysis Therapy and send it to JSDT by fax or snail mail.
